Participants in a medical research study must click the download button below, fully read and complete the form, then return an original copy of the Medical Records Release Authorization Form to Oviedo Medical Research, releasing Protected Health Information (PHI) to the researchers.

Without a signed consent form on file, participants cannot proceed as part of the study.
